Dr. J Anti-Aging Clinic — Membership Agreement
Effective upon enrollment. Please read carefully before subscribing.
1. Membership & Billing
By enrolling, you authorize Dr. J Anti-Aging Clinic to charge your payment method automatically on a recurring monthly or annual basis, depending on the plan selected. Charges begin on the date of enrollment and recur on the same date each billing period. Annual memberships are billed in full at the start of the term.
2. Monthly Credits
Each active membership includes a monthly credit equal to your plan's face value (e.g., $199 for Essential). Credits are applied to services at any participating Dr. J brand location. Credits do not carry over month-to-month and expire if unused within the billing period.
3. Reward Dollars
Members earn reward dollars on qualifying in-clinic purchases based on their tier's reward rate. Reward dollars have no cash value and may only be redeemed toward services at participating locations. Reward dollars do not expire while the membership is active.
4. Milestone Perks
Complimentary milestone treatments are earned at months 1, 3, and 6 of continuous membership. Perks must be redeemed at a participating location and cannot be exchanged for cash or transferred. Perks expire if not claimed within 90 days of becoming eligible.
5. Discounts
Members receive a discount on additional services beyond their monthly credit. Discounts apply to listed service prices and cannot be combined with other promotions unless explicitly stated.
6. Cancellation & Pause
You may cancel or pause your membership at any time by contacting our team or through your member portal. Cancellations take effect at the end of the current billing period — no partial refunds are issued. Paused memberships retain their tier benefits but do not accrue credits or reward dollars during the pause period.
7. Refund Policy
Membership fees are non-refundable once a billing period has begun. If you believe a charge was made in error, please contact us within 7 days for review.
8. Modifications
Dr. J Anti-Aging Clinic reserves the right to modify membership pricing, benefits, or terms with 30 days' written notice to members. Continued use of your membership after the notice period constitutes acceptance of the updated terms.
9. Governing Law
This agreement is governed by the laws of the State of Florida. Any disputes shall be resolved in Orange County, Florida.
By checking the agreement box and completing enrollment, you confirm that you have read, understood, and agree to these terms and conditions.
